General Responsibilities:
The school-based community health worker is a member of the behavioral health team and will be embedded in a local middle school and high school to arrange telehealth behavioral health services to students during school hours. The CHW will closely collaborate with school guidance and student support staff to identify students in need of behavioral health services. The CHW will outreach families of identified students to provide support and care coordination needed to access behavioral health services. This position will require close collaboration with students, families, medical providers and Thundermist behavioral health staff. The CHW will be expected to reduce barriers to care, address basic needs and assess for social determinants of health. This position is part time and has a grant for one year. With a possibility of the position being extended.
Required Qualifications:
1.Bachelor’s degree preferred, High School diploma and case management experience will be considered.
2. CHW certified or eligible
3 Ability to work independently and effectively in a team environment
4. Highly organized and strong interpersonal skills.
5. Maintain timely and accurate records and documentation as required.
6. Demonstrated experience working with Electronic Health Records, computers and other technology and Microsoft Office applications

Physical Effort/Environment:
- Meet with patients at the Health Center, in the community or in the home
- Flexibility to travel to meet the needs of the patient
- May require climbing several flights of stairs
- General office environment: manual dexterity needed for keyboard work, writing and phone calls
- Use of technology for home monitoring, telemedicine and other devices as needed
Work Schedule Demands:
Full-time: school hours.
Communication Skills:
Strong communication skills, both oral and written, required. - Ability to relate to patients and build trusting relationships. - Ability to communicate effectively with co-workers, patients, family/care givers, and partner agencies.
Confidentiality of Information:
A high level of confidentiality. Full access to medical records. Must always maintain all policies of confidentiality and use discretion surrounding agency information and management issues.
Significant Job Functions:
1. Key member of school-based Community Health Team; collaborates with school, physicians, other care team members, partner agencies, members of the BH team in the patient’s network of care to improve patient access; Obtain consent from outside providers involved in patient care. 2. Meet with patient/student and family as needed . Perform other duties are required.
