The RTF is a coeducational program in a warm, supportive environment that serves young people ages 12-18 who have had multiple psychiatric hospital placements or have been involved with the juvenile justice system. It is the only downstate RTF in the state that is a dual NYS Office of Mental Health/Office of Children and Family Services facility. We never consider it too late to give young people the help they need, and we are pleased to partner with these two agencies to ensure the young people in our care have the skills and support they need to achieve long-term success. Reflecting The Child Center’s dedication to comprehensive care, the RTF is staffed by clinical coordinators, psychiatric providers, a pediatrician, and 24-hour nursing staff, 24 hour youth advocates. Supports are individualized and tailored to the needs of the young people.

The Transitional Coordinator is responsible for engaging and supporting youth and families in the RTF from entry to through post-discharge from the Residential Treatment Facility (RTF).  The TC works hand in hand with the treatment team to bring the family into the center of the treatment and discharge process and secure all entitlements, resources and supports for youth and families necessary to ensure positive outcomes post discharge from the RTF.

Responsibilities:

  • Provide intensive hands-on work for a caseload of youth and discharge resources/families to ensure a successful transition into the community with a focus on integrating skills learned in the RTF into the discharge setting
  • Participate in clinical and treatment meetings including, but not limited to, pre-admissions, CTPs and discharge conferences
  • Assess family strength & needs, assuming primary responsibility for the development and coordination of plans to address impediments to successful reunification
  • Maintain a current working knowledge of local resources, referral procedures and contacts
  • Effectively and efficiently navigate and communicate with systems impacting youth and families including, but not limited to, OCFS, DSS/ACS, DOE, etc.
  • Provide hands-on work to educate and coach youth and families towards a working knowledge of community resources and supports with direct support and linkage to local resources in the child’s home community both throughout admission and post discharge
  • Responsible for the continued scheduling and progressing of family time including transportation needs, skill building and behavior management techniques for families
  • Ensures all necessary school supports and needs are met, maintaining regular contact with the RTF in-house D75 school system
  • Provide for and maintain tracking of all transitional/wrap-around funds for clients following OMH policies, ensuring they are utilized for their intended purposes
  • Ensure compliance with all OMH regulations and guidelines
  • Other tasks as required

 

 

Requirements:

  • Must be Licensed with NYS as a LMSW, LMHC or LMFT
  • 2 years of experience working within youth dealing with Psych/MH/Trauma/SED
  • Strong communication, documentation and assessments.
  • Role requires 2-3 days doing home visits.
