Hospice Intake, LPN
Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. POSITION SUMMARY
The LPN Intake Coordinator is responsible to facilitate patient/family education, evaluation and/or the admission of patients into hospice care and/or associated services. This is a full time position working Monday through Friday with a weekend rotation (every third), and is based out of the Valencia Office.
QUALIFICATIONS
- Current LPN license in the state of Pennsylvania preferred
- Two (2) years hospice experience preferred
- Possesses strong assessment, problem solving and decision making skills
- Solid computer skills, including Word and Excel
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
RESPONSIBILITIES
- Discusses hospice information with referral sources in order to assess, plan and implement timely transition a patients to hospice care
- Works closely with scheduler to meet referral source and patient/family expectations regarding Reflection of Care (ROC), evaluation and/or admission
- Completes referral information appropriately
- Obtains the necessary information for patient admission or ROC, including orders, demographic information and health records
- Obtains insurance authorizations in a timely manner
- Gathers information to assure safety of patient, family and employees
- Adheres to regulatory compliance activities and processes
- Facilitates necessary data collection and entry of patient information for physicians and Interdisciplinary Group (IDG) members; arranges the necessary IDG members’ involvement related to the admission
- Communicates accurately with coding, authorization and Inpatient Units to assure regulatory management of hospice criteria, NOEs and eligibility
- Provides information to Patient Care Coordinators (PCC) to assure management of medications
- Completes all computer and/or paper documentation requirements within the timeframes specified
- Participates in required on-call responsibilities
