Community Health Worker - West Campus
Summary |
Reports to the Director of Behavioral Health and works cooperatively with community health partners to improve patient care and health outcomes for identified and referred persons. The goal of community health services is to engage/re-engage persons with needed health and community resources. Community Health Workers will primarily be working out in the community and persons’ homes to assist persons with accessing healthcare resources.
Essential Duties & Responsibilities |
- Assist the Director of Behavioral Health in program planning and implementation.
- Demonstrate the ability to provide community services including assessment of needs, health management planning, providing information/referral about available services (within and outside of GESMV), assistance scheduling and/or transporting to appointments, and coordination of care with healthcare providers.
- Demonstrate the ability to work cooperatively with other GESMV staff and community partners (health providers, mental health agencies, housing providers, social service providers, and others).
- Performs work in an ethical manner and ensures that persons are treated with dignity and respect. Self-determination of persons must be acknowledged and respected.
- Must have excellent oral and written communication skills and be able to work with a diverse staff and client population.
- Review documentation and records to meet requirements for Pathways HUB to ensure payment for community health linkages (with assistance from Director of Behavioral Health).
- Complete first day paperwork and develop health management/care plans and goals with persons served.
- Compile necessary reports and documentation as required.
- Provide supervisor with necessary documentation and reports in a timely fashion.
- Ensure compliance with all agency policies, procedures, and safety standards throughout work areas and maintain same to meet Department of Labor and all other relevant regulatory standards.
- Travel within the community to assist persons in securing and connecting to necessary resources as required. Provide transportation with agency van and maintain adequate liability insurance.
- Ensure compliance with all Agency policies, procedures and safety standards throughout work areas and maintain same to meet CARF, Department of Labor and other regulatory standards. Responsible for adherence to all requirements related to specific funding sources. This includes maintaining necessary records and submitting reports as required.

Education Requirements |
Minimum of a Bachelor’s degree in health or social service related field required. Community Health Worker certification preferred. Education related to and experience working with persons who have experienced chronic health problems, homelessness, mental illness and/or substance abuse problems.
Experience Requirements |
Experience working cooperatively with medical/health providers preferred.
