Under the direction, guidance and general supervision of the Director of Quality and Population Health, the Population Health Manager is responsible for the direction and supervision of GHC-SCW’s population health initiatives, community partnerships, community health programs, and social work services. The incumbent provides guidance and supervision to the Population Health Department staff, functions independently, and is accountable for sound decision making and judgement. The Population Health Manager actively supports the organization’s health equity initiatives for both members and patients including social determinants of health and ameliorating health disparities where possible. The incumbent also participates in cross-departmental collaboration with Language Services, Marketing, Primary Care, and Quality. The Population Health Manager will serve as a key GHC-SCW community relations figure representing the organization in the community

Job Specific Minimum Qualifications

  1. Bachelor’s degree from an accredited college or university in Business Administration, Healthcare Administration, Nursing, Social Work, or relevant field is required, or equivalent combination of education and experience. 
  2. Ability to travel to various work-related sites is required. Employee is expected to provide personal transportation.  Possession of a valid, Wisconsin driver license is required, if the employee is operating a motor vehicle.   
  3. Minimum of three (3) to five (5) years of experience in healthcare administration with business activities related to contract administration, policy development, community outreach or other relevant activities is required. 
  4. Experience working in a managed health care delivery system is preferred.
  5. Knowledge of Medicaid regulations, processes and related procedures is required.
  6. Knowledge of NCQA and HEDIS standards and requirements is preferred.
  7. Knowledge of program management and administration including the supervision and direction of staff is required.
  8. Ability to work effectively with the managers and staff of various GHC-SCW departments and affiliated delivery systems is required.
  9. Ability to maintain organizational, personnel related and patient confidentiality. Knowledge of HIPPA requirements.
  10. Ability to pay attention to detail is required.
  11. Ability to work effectively with persons from different cultural and educational backgrounds is required.
  12. Knowledge of and ability to use Microsoft Office programs is required.                        
  13. Ability to use and/or learn electronic medical records systems such as EPIC systems.                
  14. Strong verbal and written communication and interpersonal skills are required.
  15. Excellent organizational skills with the ability to handle multiple tasks simultaneously with frequent interruptions are required.
  16. Strong problem solving and analytical skills are required.
  17. Ability to be a team player in a team-oriented environment is required. Ability to interact with staff at all levels is required.
  18. Ability to be flexible, meet deadlines, work under pressure, and manage changing priorities is required.
  19. Ability to work flexible hours is required.   
  20. Effective presentation skills, including the ability to present complex ideas to sophisticated audiences.
