Kourageous Kids CSW Position Summary: 

As a member of the interdisciplinary team, provides social work services to patients and families as assigned. Services include: assessment, emotional support, assistance with financial and other social service needs, support and guidance regarding end of life decisions, legal affairs, and family assistance in planning for care giving.

 

CSW Position Duties & Responsibilities:

  • Conducts psychosocial assessment in context of social, environmental, and functional factors of patient and family.
  • Implements psychosocial interventions identified in the interdisciplinary plan of care related to care giving, end of life decisions, advocacy, resource acquisition, nursing home placement, etc.
  • Provides emotional support to patient and family.
  • Demonstrates effective communication and relationship building skills with patient/family.
  • Provides closure and planning with patient/family at time of transfer or discharge.
  • Provides bereavement follow up after death as determined by IDG.
  • Demonstrates effective communication, relationship building skills, and coordination of care with IDG and Hosparus Health staff to achieve quality outcomes.
  • Participates in development of the interdisciplinary plan of care and evaluation of progress towards goals and interventions, and facilitates patient/family participation. 
  • Participates in on-call services and/or responds to requests for psychosocial/emotional/spiritual care outside normal business hours. 
  • Identifies roles for volunteers and other services, when appropriate.
  • Supervises interns/students, as assigned.
  • Completes psychosocial assessment in a timely manner.
  • Documents contacts with/on behalf of patient/family that are concise and reflect assessment, planning, interventions, evaluation and closure according to timeliness standards.
  • Submits all other required/requested documents within a timely manner.
  • Reflects productive and efficient practice in daily work, according to departmental goals and expectations.
  • Serves as professional resource for psychosocial related issues.
  • Demonstrates effective communication and relationship building skills with social services and advocacy leaders and agencies.
  • Provides community education and outreach upon request.

CSW Position Qualifications: 

  • Must have Master’s degree in Social Work.
  • Must be a Certified Social Worker in Kentucky or Licensed Social Worker in Indiana, as indicated.
  • One year social work experience in health care is preferred.
  • Must have a valid driver’s license in accordance to policy.
  • Use of technology-based tools, including electronic medical record, strongly preferred.

Kourageous Kids is a dedicated pediatric program under the administration of both Hosparus Health and Pallitus Health Partners. This unique program serves patients with complex and life-limiting conditions and diagnoses by providing patients and their families with medical, psychosocial, and spiritual support to help manage symptoms, reduce stress and anxiety, and enhance quality of life. In addition to providing palliative care support to any pediatric patient facing a serious or life-limiting illness, the team also provides hospice support to patients and their families that have a prognosis of 6 months or less.  This team helps advocate for patients and families within the larger healthcare system, provides education and empowerment, and walks with them through difficult conversations and decision-making processes. This position requires flexibility, compassion, and a commitment to collaboration and teamwork. While the work can at times be emotionally heavy, this team is dedicated to supporting one another and finding joy and levity when times are difficult. The Kourageous Kids team strives to go above and beyond in providing excellent care to patients and families, resulting in work that is simultaneously challenging, meaningful, and rewarding.
