Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals.
  Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Prototypes Women's Center is a 180 bed residential facility for women (with or without children) that also offers intensive outpatient services to adult men and women. We provide comprehensive substance use, mental health, and psychiatric care by an interdisciplinary team of professionals. Treatment episodes range in time from 60 days to 6 month. Prototypes is a fast-paced environment that strives to promote optimal client care and promotes self-sufficiency while ensuring safety and shelter to those in need. 

The SUD Care Coordinator is a registered or certified substance use disorder professional who maintains registration from an approved/accredited California agency. The SUD Care Coordinator is responsible for delivering services to a caseload of clients and providing individualized substance use disorder services. The SUD Care Coordinator works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with

 

HealthRIGHT360's philosophy, goals, policies, mission and vision. The SUD Care Coordinator may provide treatment utilizing different modalities including individual and/or group. Services may be provided in outpatient or the field based on client and program needs. 

 

Key Responsibilities

  • Assists clients with intake, assessment, and orientation to program upon admission or transition in care level.
  • Helps lead the client through the treatment plan and delivers individual and group substance use disorder services.
  • Monitors client's progress toward achieving treatment plan goals and provides treatment plan input.
  • Utilizes trauma informed care while applying motivational interviewing, cognitive behavioral therapy, and evidence-based practices.
  • Seeks clinical consultation throughout the treatment episode and when encountering new and/or high risk clinical circumstance. 
  • Works collaboratively with other members of the treatment team including the Case Manager, Clinical Therapist, Medical, and Psychiatrist.
  • Maintains frequent communication with the treatment team and engages in regular consults, clinical team, case conferences, and plan development meetings.
  • Delivers services in outpatient or field depending on assignments and client need.
  • Writes and completes all progress notes within 72 hours of service delivery.
  • Maintains compliance with due dates for all clinical documentation.
  • Writes client progress letters and court reports and obtains final approval from the supervisor. 
  • Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards.
  • Meets expected client care hours (productivity expectation is 20 client care hours per week).
  • Responsible for treatment oversight of the clients on their caseload, including case management services, individual counseling and urinalysis testing. 

Education and Knowledge, Skills and Abilities

  • Registration as Substance Abuse Counselor from an approved/accredited California agency.
  • Preferably a minimum of two years experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery. 
  • Experience working with clients experiencing acute withdrawal from substances.
  • Knowledge of and experience with providing trauma informed services.
  • Experience delivering evidence-based practices.
  • CDCR fingerprint clearance.
  • Must not be on parole or probation for a minimum of two years.
