The Data Coordinator will be working at Prototypes, Pasadena Outpatient Behavioral Health Services site. The Data Coordinator role is to work as part of a treatment team with a targeted approach to administer and quantify treatment outcomes through periodic interviews with clients to complete outcome measures required as part of the grant & public funding requirements of the program. The primary responsibilities of this role are focused on client interactions through interviews at the beginning of a client's treatment episode, at a mid-point of their treatment episode, and at the end of their treatment episode to identify changes in their behavioral health symptomatology, effective treatment services, and overall program outcomes. Additionally, the Data Coordinator will collaborate with treatment team members to coordinate administration of outcome measures and will complete data entry of outcome measures into related data entry portals. 

Key Responsibilities

Direct Service: 

  • Engage with clients at distinct intervals (intake, 6-month mark, discharge) in client treatment episodes to complete clinically and culturally appropriate interviews.
  • Document interviews (and non-interview, when interviews are not appropriate) resulting in completion of outcome measures within 2 business days of intake commencement.
  • Complete data entry of outcome measures data into related portals. 
  • Ensure timely and appropriate billing documentation as required.
  • Seek clinical consultation when indicated. Seek clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstance.
  • Work collaboratively with other members of the treatment team including the Therapist, Occupational Therapist, Case Manager, and Psychiatric/Medical Team.
  • Deliver services in physical location deemed optimal for interfacing with client/caregiver depending on assignments and client need.
  • Assess the client’s case management needs and complete releases of information as necessary.
  • Support client in connecting with resource and linkage needs when possible. 

Clinical Documentation:  

  • Write and complete all progress notes in standard with agency guidelines after service delivery.
  • Maintain compliance with due dates for all clinical documentation.
  • Write client progress letters and court reports and obtain final approval from the supervisor.
  • Documentation must be maintained in compliance with the agency’s policies and procedures, HIPAA, 42-CFR, DMH, and SAPC standards.
  • Complete all assigned URCs in a timely matter. 

Administrative:

  • Maintain full compliance with requirements for certification. 
  • Perform general administrative tasks: data entry, scanning, filing, organizing, faxing, mailing, population management, assisting and directing calls, clients and visitors to the appropriate staff, groups and/or departments, and checks in clients for appointments and groups, assists with Health and Safety items and drills.
  • Actively participate in agency and team meetings.
  • Participate in training opportunities and complete all assigned training in a timely manner.

And perform other duties as assigned.

Education and Knowledge, Skills and Abilities

Education, Certification, or Licensure:

  • Bachelor’s degree in related field preferred.
  • Possess current First Aid and CPR certification or ability to obtain within 30 days of employment.

Experience:

  • Two years experience in data collection and entry preferred.
  • Two years experience in administrative support role preferred.
  • One year experience in mental health setting preferred.
  • Experience working with children and caregivers preferred.
  • Spanish-speaking strongly preferred.

Background Check and Other Requirements:

  • Must not be on active parole or probation.
  • Ability to obtain and maintain satisfactory background check.
  • Must be able to pass health screening and TB test requirements. 
  • Must be able to meet and maintain credentialing requirements.
  • Must be fully vaccinated against COVID-19, including booster shot. Medical or religious exemptions available.
