Job Overview
 
Prototypes S. Mark Taper Family Living Center is a 32-unit apartment complex that shares a campus with Prototypes Residential Substance Abuse Treatment Program and Prototypes Outpatient Program. The Housing Program at S. Mark Taper Family Living Center serves women and women with children with Substance Abuse and Mental Health Disorders. The Housing Program also services 64 scattered-site clients living around the Pomona area. The Housing Program provides case management services to help clients work toward acquiring permanent housing. Prototypes Housing Program is a fast-paced environment, as we strive to remain on the cutting edge of service provision.

Prototypes Pomona Behavioral Health Bridge Housing, a HealthRIGHT 360’s program, provides interim housing supportive services for people experiencing homelessness including: linkage to recovery services, medication support, case management, crisis management, and transportation services/resources. 

The Specialized Case Manager will support clients enrolled in Housing for Health Bridge Interim Housing Program by assisting clients with building daily life skills, budgeting skills, and other housing related skill sets to prepare for transition to permanent housing.

Key Responsibilities

Direct Service:

  • Conducts a needs assessment that includes basic physical and social needs.
  • Identifies goals and documents individual Service Plans.
  • Provides follow-up to ensure clients are moving toward goals, as per Service Plan.
  • Provides One-on-one Case Management sessions at minimum 1x per week. 
  • Supports client when dealing with interpersonal conflicts with roommates/others.
  • Conducts/participates in House Meetings to support clients and promote a safe and amicable environment in the shelter.  
  • Encourages client to be engaged in program and program demands.
  • Supports and guides client in placing work orders when needed for Property Manager.
  • Helps develop transition plans, including assistance with housing, aftercare services, etc.
  • Provides community referrals based on individual needs/requests.

Documentation:

  • Utilizes electronic record systems as per Prototypes and contract requirements. 
  • Documents One-on-One Case Management sessions with assigned clients in a timely manner.
  • Creates Service Plan as required by contract.
  • Creates/guides and supports the creation of Behavioral Support Plans for clients with behavioral problems.
  • Documents communications with internal and external agencies to ensure consistency in services.  
  • Participates in program audit preparations

Community Relations:

  • Makes appropriate referrals and follows up to assure delivery of services.
  • Attends any contract required trainings and meetings. 
  • Attends house related coalitions/groups to support programs efforts.

Administrative:

  • Supports and coordinates with client’s external services/agencies.
  • Supports housing navigation process.
  • Responsible for coordinating case conferences with external and internal agencies associated with case load.
  • Informs external and internal agencies/programs about client’s progress/lack of. 
  • Participates in staff meetings, team meetings and in-service trainings.

And perform other duties as assigned.

Education and Knowledge, Skills and Abilities

Education, Certification, and Experience:

  • Bachelor’s degree in related field.
  • First Aid certified within 30 days of employment.
  • CPR certified within 30 days of employment.
  • Minimum of two years’ experience in a supported housing environment.
  • Experience working with substance abusers, chronically homeless, and/or reentry populations.
    Background Clearance 
  • Must not be on active parole or probation, for a minimum of two years.
  • Ability to obtain and maintain satisfactory background check.
  • Ability to obtain and maintain live scan clearance.
  • Must be fully vaccinated against COVID-19, including booster shot. Medical or religious exemptions available.

Knowledge:

  • Knowledge of gender-responsive, trauma-informed, and co-occurring treatment.
  • Knowledge of clinical documentation (treatment plans, progress notes, etc.).
  • General understanding of Housing First and Harm Reduction models.

Skills and Abilities

  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and Internet applications.
  • Excellent verbal, written, and interpersonal skills.
  • Integrity to handle sensitive information in a confidential manner.
  • Excellent organization skills and ability to multitask and juggle multiple priorities. 
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.
  • Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.
  • Bilingual preferred.
