Job Overview

This is a union position.
 

Prototypes, a program of HealthRIGHT 360’s residential substance use disorder (SUD) program is a new program contracted with Department of Public Health’s (DPH) Substance Abuse Prevention and Control (SAPC) program for residential drug Medi-Cal (DMC) services for up to 99 adult men and women: 33 that are for men who are judicially involved, 33 for men, and 33 for women. The residential DMC program will employ 100 people and works closely with the other Behavioral health center programs to provide a continuum of services for low income/Medi-Cal population.
The program is located on the campus of Martin Luther King Jr. Hospital in the Willowbrook area of South Los Angeles and will serve all LA County residents.


Responsible in assisting Nursing Supervisor, Nurse Practitioners, and program Directors in the following area: medication management and education, delegate duties/tasks to Nurses, chart audits and supports daily operations of residential treatment center. Utilize medical expertise to provide services and support as part of a treatment team and assist clients in their recovery. Responsible for providing medication management services to consumers receiving services at residential treatment center. The LVN works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission and vision. The LVN is customer service focused and is quick to take the lead in providing crisis intervention and participant stabilization. The LVN has a high degree of self-awareness and the ability to stay calm under pressure.

Although all LVNs will have an assigned main responsibility, they will be expected to be cross trained and cover these other departments on an as needed basis based on work load and coverage need. LVNs will be expected to cover weekends as well as evenings depending on their shift assignments.


The LVN will be cross trained in the following duties:

 

  • Medication/Floor LVN will help assist with keeping track and storing medication for clients at the residential facility and help clients with taking their medications during med line. They will also be expected to monitor and notify medical personnel of medication adherence issues / side effects that clients may be experiencing. Medication/Floor LVNs are also expected to help clients with making outside medical appointments and support ancillary floor staff as needed. LVNs within this department will work a block shift schedule and will need to cover evenings and weekends.
  •  Intake LVNs will be responsible for helping with medical evaluations for new clients coming into the facility and documenting their findings. They will help coordinate with other intake staff and other licensed medical providers to ensure that incoming clients are medically stable enough to initiate residential treatment. If incoming clients require further stabilization, they will work with staff to ensure patients are transferred to appropriate withdrawal management/detox facilities. Intake LVNs are expected to work AM and PM shifts.
  •  Clinic LVNs are responsible for working closely with the nurse practitioners who are responsible for providing ongoing mental health services, medication assisted therapy services as well as urgent primary care services for clients at the facility. They will help with documenting vitals as well as rooming clients prior to their assessment with Nurse Practitioners. They will work closely with the Nurse practitioners to help with scheduling, communicate medication prescription changes to the Medication/Floor LVNs and coordinate with other LVNs to communicate relevant medical updates to the Nurse practitioners. They will also be expected to help set up and facilitate zoom meetings for interdisciplinary huddles every morning. Clinic LVNS are expected to work AM and PM shifts.

 

Key Responsibilities

General Responsibilities:

  • Assist the nursing supervisor and associate medical director in the day to day monitoring, treatment and maintenance of client medical conditions as qualification and license allows.
  • Administratively, takes the lead with the following: MAR binders, transportation request, client appointments, assist with blood draws, running Pop Sheet reports and manages OTC box.
  • Intake process: Assessment (head to toe), COVID swab, TB test, medication pass, and medical triaging.
  • Effectively communicates with treatment teams, nurses and program staff.
  • Facilitate health, wellness, and medical education for staff and clients.
  • Work as an active team member and closely collaborate with team members.
  • Interface with multidisciplinary team in treatment planning and service delivery.
  • Establish working relationships with community partners for continuity of care.
  • Responsible for tracking and ordering medical supplies.
  • Develop and assess effectiveness of individualized treatment plans and consumer progress.
  • Serve as care coordinator for clients and completion of all coordinated care documentation.
  • Assist clients develop independent living skills to promote independence and self-sufficiency.
  • Arrange work schedule in accordance with agency’s needs.
  • Comply with the agency’s policies and procedures.

Perform other duties as assigned Documentation:

  • Conduct chart audits and report findings to Medical Management and Nurses.
  • Ensures that MARs are complete and accurate.
  • Submit program and contractual reporting and documentation requirement in a timely manner.
  • Maintain documentation in compliance with agency, HIPAA and DMC/SAPC standards.
  • Comply with all documentation standards and procedures set down by law or funding agency requirements.
  • Maintain up-to-date files and timely documentation in Welligent. Work collaboratively as part of treatment team.
  • Meet expected performance standards as assigned by Supervisor.
  • Monitor and document for medical quality assurance.

When providing direct services, complete all required paperwork, charting, records maintenance in a timely manner Administrative:

  • Answer’s telephone and responds to inquiries regarding residential treatment facility within 24 hours.
  • Responsible for signing off medications, counting controlled medications, reading log entries for up-dates, and recording all changes.
  • Maintains supplies for urine and oral fluid drug testing.
  • Collects, organizes, and reports results of daily urinalyses.
  • Maintains stock of OTC medications and First Aid supplies.
  • Acts as liaison for removal of bio hazardous waste.
  • As needed, assists Office Assistant with duties of checking in visitors and monitoring visits, ensuring participants sign in/out with a valid pass, monitoring participants when they return from off-site passes, and checking all bags upon entering the facility.
  • Responsible for signing emergency passes when primary counselor is out of the office.
  • Performs periodic facility checks to ensure and maintain the safety and security of the facility.

Training, Supervision, and Quality Improvement:

  • Actively participates in agency and team meetings.
  • Participates in training opportunities.
  • Communicates collaboratively with all members of the behavioral health team including: medical, mental health, substance use disorder, and other staff.
  • Completes all assigned trainings and Relias trainings in a timely manner.

Professionalism and Other Duties:

  • Responds to requests for coordination of facility needs from Supervisor and other Leadership.
  • Attend regular staff meetings and other meetings as assigned.
  • Reads emails daily and responds when appropriate.
  • Transports clients as needed.
  • Conducts random and planned room searches primarily in the withdrawal management units.
  • Completes random and scheduled client UA tests and documents test results.
  • Comply with agency’s policies and procedures.
  • Meet expected performance standards as assigned by supervisor.
  • And, other duties as assigned.

Education and Knowledge, Skills and Abilities

Education and Experience Required:

  • Licensed vocational nurse in California.
  • IV certification.
  • Valid CA Driver License and automobile insurance.
  • Active LVN licensed.
  • Minimum of 1 years community based program management experience or equivalent.
  • Knowledge of trauma preferred.
  • Commitment to providing nonjudgmental quality health care to those in need.
  • Experience serving seriously and persistently mentally ill clients with co-occurring challenges.
  • Experience and commitment to working with diverse communities.

Desired:

  • Active LVN with minimum 3 years of experience.

Background Clearance Required:

  • Department of Justice background clearance.
  • Must not be on parole or probation for a minimum of two years.

Knowledge Required:

  • Knowledge issues of related to substance abuse, mental health, and criminal background.
  • Culturally competent and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.

Other Requirements:

  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.
  • A valid California driver’s license and current automobile insurance.
  • Must be vaccinated and boosted (when eligible) against COVID-19.

Desired:

  • Bilingual in English/Spanish.
  • Bachelor’s Degree in related field.
  • One year of experience in a residential substance abuse treatment setting or a withdrawal management/detox setting.
  • Knowledge of co-occurring disorders and trauma informed treatment.
  • Knowledge of Clinical documentation (treatment plans, progress notes etc.).
  • Experience working with criminal justice population.
