Job Overview

The Nurse Manager is a key part of medical leadership in San Mateo county HealthRIGHT 360 medical settings, and as such performs clinical and administrative tasks consistent with HealthRIGHT 360 policies and protocols.  They play a key role in ensuring efficient patient flow, access, and satisfaction; spear-heading continuous quality improvement efforts; supervising, supporting and coaching support staff (i.e., medical assistants and other nurses); and acting as a point person for external collaborators for operational issues.  In addition, they will specifically manage activities associated with the data management, vaccine management, laboratory, hazardous waste disposal, and the supply chain. 

At all sites, they will provide primary care nursing support to patients and develop/implement/monitor systems that support that care.  They recognize the challenges faced by patients with substance use disorders and/or serious mental illness and utilize a trauma-informed, motivational, and client-centered approach in their work. They will assist the medical provider with direct patient care during office hours.  Outside of office hours, they will work closely with the Telecare team at Cordilleras MHRC and the medical assistants at the MAT clinic to support care (e.g., reviewing lab results, facilitating referrals to specialty care and imaging, being the point of contact for follow-up questions, etc.).

They are expected to provide all patient care in a sensitive and non-judgmental manner, to maintain a high degree of professional competence and the highest possible ethical standards, and to function as part of a care-giving team. They are also responsible to uphold and imbue in others a commitment to addressing racism, and gender and sexual discrimination within the organization and our clinical practice.

Key Responsibilities

Clinical Care Responsibilities:
  • Provides nursing care, within the scope of license and agency credentials, to residents of the Cordilleras Mental Health Rehabilitation Center and the San Mateo MAT Clinic according to pre-arranged schedules, documenting care promptly and following charting protocols and applicable CA state licensing requirements.
  • Utilizes trauma-informed, motivational interviewing skills and encourages patient activation and self-management.
  • Facilitates team functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions (e.g., rooming, vitals, etc.) to ensure the completion of patient sessions in a timely and thorough manner.
  • Administers vaccines and therapeutic/diagnostic medications via injection.
  • Reviews lab results and ensures appropriate follow-up of abnormal results through the medical provider.

·        Cordilleras MHRC

o   Takes responsibility for patients with complex medical issues, facilitating referrals and communicating plans with the Telecare team.

o   Ensures return of all ordered/performed testing and consultations.

o   Follows-up with all patients and the Telecare team regarding recent medical emergency room visits and medical inpatient stays.

o   Participates as an active member in case-conferencing and other collaborative work with Telecare; works collaboratively with Telecare team to facilitate integration between primary care and mental health to improve patient outcomes.

·        San Mateo MAT Clinic

o   Provides care through shared medical appointments to new patients and patients receiving injectable medications.

o   Assesses and provides nursing advice to patients who walk-in with medical complaints or who call-in with medical concerns seeking advice.

o   Responds to patient requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.

Obtains and maintains licensure, and BCLS/CPR certification.

Attends continuing professional education programs, at the request or with the approval of supervisor, demonstrating participation in 30 hours continuing professional education every 2 years.


Leadership Responsibilities:
  • Supervises medical assistants, nurses, and case managers, providing training, ongoing recognition, feedback, and support to develop team members’ professional skills and incorporating constructive feedback to support behavior change.
  • Participates in the development and implementation of team-based workflows.
  • Demonstrates comfort in changing systems, champions change, and leads change processes.
  • Demonstrates commitment to mission of the organization, queer health, racial justice, and community-oriented primary care.
Performance Improvement and Quality Assurance Responsibilities:
  • Demonstrates comfort in changing systems, champions change, and leads change processes.
  • At Cordilleras MHRC, develops and implements systems that ensure appropriate chronic and preventive care for all patients residing on the unit, whether or not they have been seen by the medical team.
  • At San Mateo MAT Clinic, meets benchmarks for applicable quality indicators as noted in the annual quality management plan including clinical outcomes, productivity standards, cycle times, no-show rates, and landing times.
  • Develops and implements systems that ensure that clinics are meeting contractual and regulatory requirements.
  • Along with colleagues, assists in continuous quality improvement through on-going problem/needs identification and problem solving.
Administrative Responsibilities:
  • Manage relationships with external vendors and partners including hazardous waste disposal, laboratory, specialty pharmacies, the San Mateo County Immunization Program, the IMAT Program, San Mateo Medical Center and Telecare. 
  • Monitor supply chain and ensure adequate inventory, ordering as needed. 
  • Monitor equipment and ensure that equipment is in good working order.
  • Ensure collection and reporting of data as needed to meet regulatory and funding criteria.
  • Ensure faxes and mail are appropriately distributed.
  • Ensure that documents, telephone encounters and labs are resolved in a timely way by all support staff.
Organizational Responsibilities:
  • Keeps up to date with operational and procedural requirements.
  • In conjunction with leadership, assures organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the clinics.
  • Supports the interpretation of standards and processes to meet best practice standards established in the health services program.
  • Adheres to company dress code and maintains professional appearance.
  • Completes mandatory trainings within deadlines.
  • Maintains availability for various administrative/clinical duties as requested by supervisor (e.g., case presentations; in-service training to staff; represents HealthRIGHT 360 at and/or attends professional conferences; leadership/consultation to committees; attendance/speaking at fundraising/community events, etc.).

And, other duties as assigned.

Education and Knowledge, Skills and Abilities 

Education and Experience Required:
  • Graduation from an accredited school of nursing.
  • Possession of a valid California license as a registered nurse.
  • Possession of BCLS/CPR certification.
  • Minimum of 2 years management experience.
  • Two years of direct patient care experience in ambulatory care and/or a community health center.
Desired:
  • Experience in quality management and performance improvement.
  • Experience working successfully with issues of mental illness, substance abuse and criminal backgrounds.
  • Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and culture backgrounds and economic status.          
Background Clearance Required:
  • Must not be on active parole or probation.
Knowledge Required:
  • Knowledge of co-occurring disorders and trauma informed care.
  • Strong proficiency with Microsoft Office applications, specifically Word, Excel, Outlook and internet applications.
Desired:
  • Bilingual Spanish/English.
  • Experience with electronic medical records.
  • Comfort working on teams.
