Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals.
  Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Pioneer Ct is an Outpatient substance use disorder program located in the city of San Mateo serving adolescents and adults.  The team consists of Care Coordinators, Clinical Therapists, an Outreach Coordinator, a Compliance Coordinator, Administrative Support Staff, and Intake Coordinator and more. Services provided include Education and Outreach, Screening/Intake/Assessment, Individual and Group Counseling, Case Management/Care Coordination, Crisis Intervention and pro-social activities.

Utilizing a gender-responsive, trauma-informed approach, therapists have clinical responsibilities and LPHA (Licensed Practitioner of the Healing Arts) assessment responsibilities. This position will be able to carry a caseload of individual and group clients in the outpatient substance use disorder program. The Therapist will provide individual and group therapy/counseling and collaborate with other providers, both internal and external, to HR360 to assist clients with meeting their treatment plan goals and participate in discharge planning. In addition, this individual will provide crisis management as needed and handle all necessary follow-ups. The Therapist will administer intake and admission assessments, create comprehensive treatment plans/problem lists, and sign off on other required documentation, including diagnosis and medical necessity.  The clinical therapist will work in the clinic office and may be asked to work in the community, in home or in schools; in addition, this position requires working Saturday mornings on a rotating basis and evening hours as required for client need/access to treatment. This position's expected weekly service delivery/productivity standard is a minimum of 50%, which is 20 direct billable client hours.

Key Responsibilties:

-Provides weekly individual therapy/counseling to clients participating in an outpatient substance use disorder program.
- Care coordinates with treatment team and support services
- Facilitates group sessions such as CBT, Seeking Safety, Co-Occurring Disorders, Gender Specific, Relapse Prevention, Education, etc.
- Completes intake/assessments and determines medical necessity
- Documentation of services in agency electronic health records
- Must be willing to work one evening per week and to cover Saturday morning groups on a rotating basis

Position Requirements:

- Licensed or Board Registered LCSW or ASW, LMFT or AMFT, LPCC or APCC
- Experience working with clients with substance use and/or co-occurring disorders
- Knowledge and experience working with criminal and/or juvenile justice populations
- Threshold language in Spanish or Asian languages preferred
