General Summary:  Mid-Level Practitioner provides comprehensive, coordinated primary and preventive health care to patients. This staff will interact as an agent with outside entities on occasion. Required to be familiar with, abide by, and enforce State of Arizona and contract mandates, obligations, deliverables, and company regulations. Coordinates patient needs with other resources, agencies, and / or other organizations providing behavioral health services to AHCCCS patients. Will provide documentation on all work performed into the electronic health record. Provide comprehensive primary care for adults.   May serve as preceptor to residents, medical students, and advance practice nursing students. Attend monthly provider meetings.  Remain current with standards of practice. Perform other duties or special projects as assigned by medical director.  Occasional travel to other areas of Arizona required. 

JOB RESPONSIBILITIES:

  • Serves as the family nurse practitioner to assigned members. 
  • Develops differential diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, patient education, and records progress towards treatment goals.
  • Completes medication education and informed consent when possible.
  • Documents all activities in the electronic health record according to policy.
  • Assesses members’ needs and guides the clinical team while managing treatment plan updates.
  • Initiates contact with outside Providers according to established guidelines.
  • Works closely with other personnel to ensure that each treatment modality is appropriate for the member in addition to working with other healthcare providers involved in the overall care of the recipient of care.
  • Responsible for following any policies, procedures, and controls established by the organization, the HIPAA Privacy Officer, and/or the HIPAA Security Officer regarding access to, protection of, and the use of personal health information (PHI). 
  • Performs other duties as assigned or necessary as it relates to the general nature of the position.

 

QUALIFICATIONS:

  • Experience – 2 years of experience in certified field.
  • Certification- Valid DEA certificateActive NCCPA certification (PA-C)
  • Licensure – Current Arizona License to Practice Medicine. Licensed as Family Practice or Internal Medicine, Valid DEA license; DEAX license preferred.

 

REGULATORY:

  • Minimum 21 years of age.
  • Arizona Driver’s License, 39-month Motor Vehicle Report and proof of vehicle registration and liability insurance that meets insurance requirements, if required for position.
  • Must possess and maintain Level 1 fingerprint clearance throughout employment.
  • CPR and First Aid certification, if required (Employer provides).
  • Initial current negative TB test result, if required (Employer provides).
