General Summary: The Lead Direct Service Provider is responsible for providing support, direction, and assistance to the team, as well as coordination of day to day functioning. Provides direct care and supervision for vulnerable adults (members) in Residential Group Homes and/or Day Program settings and in the community. Each person employed in the Chino/Prescott ICHD DDD program will be trained to work at any of our three locations.
 
JOB RESPONSIBILITIES:
  • Provide support to team members and ensure duties are being completed.
  • Assign daily tasks and client ratio assignments.
  • Monitor completion and accuracy of documentation in accordance with contractual standards.
  • Assists in the development and implementation of activity plans for members who are participating in agency programs.
  • Works with families and members to achieve the goals described in the individual service plan.
  • Ensures continuity of member’s program through program data sheets, medication administration records, community outing sheets, incident reports and other documentation.
  • Maintains a daily log of pertinent events and information to be communicated with other appropriate staff and professionals; completes other documentation as required or requested by supervisor in charge of setting.
  • Maintains member files in accordance with contractual standards.
  • Monitors self-administration of medication.
  • Maintains daily physical condition of agency vehicle, to include daily mileage logs and maintenance logs to be submitted to supervisor per time scale given (only 21 year or older staff may operate ICHD vehicle)
  • Follows agency purchasing and accounting procedures.
  • Completes all training required by licensing and/or agency, or as requested by supervisor.
  • Maintains an approved schedule and acceptable level of attendance.
  • Performs other duties as assigned or necessary as they relate to the general nature of the position.

QUALIFICATIONS:

  • Education – High School diploma or GED preferred.
  • Experience – Two years of related behavioral health experience required.
  • Language – Bilingual verbal English/Spanish skills desirable
REGULATORY:
  • Minimum 18 years of age
  • Arizona Driver’s License, 39-month Motor Vehicle Report and proof of vehicle registration and liability coverage to meet insurance requirements.
  • Must possess Level I fingerprint clearance.
  • CPR and First Aid certification (Employer provides).
  • Initial current negative TB test result (Employer provides).
