RN Case Manager- PRN

Continuum Care Hospice of Michigan, an affiliate of Legacy Health Services, is looking to hire an experienced and dedicated Registered Nurse to work with our team of professionals. The ideal candidate will be a Registered Nurse with at least two (2) years of hospice experience preferred, but not required

Summary

The Registered Nurse is responsible for the holistic nursing care of the patient, and a member of the interdisciplinary team, provides for the total physical, psychosocial, emotional and spiritual needs of the patient and family.

Experience

• At least two year of nursing experience required
• Hospice experience preferred but not required
• Strong clinical assessment skills
• Ability to work as part of a team and independently in patient/family home
• Education/Licensure
• Must be a graduate of an accredited school of nursing
• Current nursing license with the State of Michigan
• BSN preferred but not required
