Enrollment Specialist

Children’s Long-Term Support Waiver

Children Youth and Family Services– Coggs location

Hourly Range: $22.57- $30.99

 

The Children’s Long-Term Support (CLTS) waiver program is a Medicaid Home and Community Based Waiver program which provides services for children with disabilities. The waiver program allows Wisconsin to use Medicaid funding for community-based supports and services to promote inclusion, access, and empowerment. The program is administered by Milwaukee County Department of Health and Human Services, Children, Youth and Family Services for residents of Milwaukee County.

Job Summary: Under the direction of the Supervisor, the main purpose of the Enrollment Specialist is to provide support to the CLTS program, Support and Service Coordination (SSC), and the Behavioral Health Services Children’s Intake team. This position will also support participants enrolling to the CLTS program through internal SSC or a contracted agency. The Enrollment Specialist will provide case management for children and families currently on the wait list.

 

Job Responsibilities

  • Create electronic file for participants, maintain waitlist, reports, and tracking. Collaborate with the Behavioral Health Services Intake team to review paperwork, Medicaid eligibility, and to determine if a participant is vulnerable.
  • Collaborate with Intake and CLTS team to ensure submissions and information are accurate. Work with internal and external SSC agencies to enroll and disenroll participants. Determine the need for Waiver Medicaid and conduct home visits as needed.
  • Enter case notes and participant information into the Program Participant System as well as Eligibility and Enrollment Streamlining. Ensure to comply with State of WI CLTS Waiver Manual requirements and meet internal quality assurance benchmarks.
  • Monitor, report, complete projects, and special assignments by establishing objectives, goals, and outcomes. Assist in the development of policies and procedures.
  • Determine outcomes and discuss services with families to make referrals. Complete purchase order requests, respite care grants, assist with home modification, as well as providing resources. Participate in trainings and meetings.
  • Perform other duties as assigned. 

 

Minimum Qualifications:

  • Resident of the state of Wisconsin
  • Valid Wisconsin driver’s license with access to an insured and reliable vehicle; both maintained throughout employment
  • Bachelor's Degree or higher from an accredited college or university in the field of Social Work, Psychology, Human Services, or in a closely related field
  • At least one (1) year of experience working with Children with Disabilities, case management, or related experience.
  • Advanced knowledge of all related computer and software applications, such as word processing and spreadsheets.

 

Additional Knowledge, Skills, and Abilities: Advanced analysis skills, problem solving skills, ability to learn Third Party Claims Administration and authorizations; ability to identify other programmatic supports, informal and formal, to support families. HSW Children's Program staff should have competencies which demonstrate they understand child and life span development, dynamics of family relationships, family support, and parenting education (e.g., principles methods and approaches, parent guidance and nurturing principles, child and family health and safety concerns, concepts of diversity in family systems, and relationships among family, school, and community). Skills needed for success in this role include customer service, excellent verbal communication, patience, writing succinctly, critical thinking, and a professional approach. HSWs must have the ability to work collaboratively with other units and organizations within and throughout the county.  Proficient in common office suite computer programs; data collection, analysis and in the use of general office equipment.
