Integrated Service Coordinator - CARS

Behavioral Health Services

Hourly Range: $25.72 to $38.82

 

Job Summary:  Under the direction of the CARS Director, the Integrated Service Coordinator will oversee the mental health residential programming offered as part of the Community Access to Recovery Services (CARS) provider network. The position will additionally be responsible for oversight of access quality, intake, and referral to CARS mental health programming.  Maintaining strong community and provider relations will be essential to the success of this role.  The Integrated Services Coordinator will provide support to the mental health group homes and adult family homes (AFH). Work hours for this position consist of regular business hours Monday through Friday.

 

The ideal candidate for this role is an individual with a passion for researching and implementing new and innovative models of residential care for individuals living with a serious and persistent mental illness.

 

Job Duties: 

  • Assist with Research and Development of new strategies. Work with the CARS team to research, develop, and implement evidence-based practices and residential models of care for individuals living with mental health disorders, substance abuse disorders, or co-occurring disorders.
  • Network Development. Work with Behavioral Health Services Network Development Specialist to build and maintain a robust and high functioning network of mental health residential providers. Assist with Request for Proposal or Expression of Interest processes when needed.
  • Provide Oversight and Management of Contracted Services. Manage program implementation and ongoing maintenance for Community-Based Residential Facilities (CBRF) and AFH programs. Serve as the primary point of contact for network providers. Facilitate operations meetings with provider agencies.
  • Evaluate and Ensure Quality of Programs. Provide assistance with program evaluations, adherence to DHS 83, and quality oversight of contracted agencies. Promote and implement continuous quality improvement projects for the CBRF/AFH network as needed. 
  • Manage Access, Referrals, and Monitoring. Collaborate with CARS team members to ensure individuals can access mental health residential services. Provide oversight to ensure high quality services, appropriate level of care, length of service, and that transition plans occur.
  • Clinical Consultation. Collaborate with CARS Administrative Coordinator and agency providers to engage in clinical consultation and resolve opportunities for individuals with high risk or complex needs.
  • Serve as Point of Contact. Maintain communication with consumers, family members, guardians, and referring agencies such as hospitals and state institutes.
  • Perform all other duties as assigned. 

 

Minimum Qualifications:

  • Current Resident of the State of Wisconsin
  • Possession of a valid Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment.
  • Master’s degree or higher in Psychology, Social Work, Nursing, or Human Services from an accredited college/university is required.
  • At least three (3) years of practical work experience in behavioral health disorders is required.
  • At least one (1) year of behavioral health program management is required.

 

Knowledge, Skills and Abilities:  Knowledge of mental health and substance abuse diagnoses and services; Knowledge of Federal, State and County regulations, Wisconsin Administrative Code for community services; knowledge of the principles and practices of developing and implementing mental health and substance abuse programs; knowledge of community-based programming for adults with mental health and co-occurring disorders; oral and written communication skills; report writing skills; analytical skills; organizational skills; ability to work productively with representatives of the Behavioral Health Services and contracted agencies; and ability to work effectively and harmoniously with staff.
