Salary Range: $61.20 - $72.00 per hour.
Shift: Fridays, 8:00am to 4:00pm (varies)

Job Summary:
The Baker Victory Healthcare Center provides high quality care to our patients. The Nurse Practitioner will be responsible for the evaluation and management of pediatric patients with a range of behavioral and developmental differences including but not limited to developmental delay, intellectual disability, autism spectrum disorders, ADHD, genetic disorders and sleep problems. This position works in collaboration with the attending physician and works within their scope of practice to provide excellent clinical care for patients. Plays a critical role in the patient family medical team.
Essential Job Duties:
1. Conducts comprehensive health history, physical assessments and plans for infants, children and young adults with acute and chronic health conditions. Utilizes advanced assessment skills to manage a cohort of patients.
2. Orders and prescribes pharmacologic and non-pharmacologic interventions, and performs other therapeutic or corrective measures to treat illness and improve health status.
3. Formulates family centered plan of care; performs daily documents as necessary and collaborates and coordinates care with other team members.
4. Collects data and analyzes when necessary.
5. Utilizes the most up to date and evidence based practice for plan of care and able to alter the plan according to family needs and effectiveness or plan.
6. Serves as an educational resource to patient/family/ provider team and other health care teams as needed.
7. Orders and administers diagnostic tests
8. Documents and update patient records; maintains accurate and timely patient records and provide documentation for insurance companies
9. Refers patients to specialists as needed
10. Provides Administrative staff with needed documentation to ensure credentialing in a timely manner
11. Create positive first impressions for patients by welcoming all patients in a warm, friendly manner, presenting a professional image, and anticipating specific patient needs such as hearing impairments, language barriers and disabilities
12. Treat everyone with respect by putting the patient and family first and respond to the patient’s needs in a timely fashion; Model the OLVHS agency values of integrity, respect, accountability, and teamwork
13. Communicate compassionately and effectively by taking time to be a good listener, trying to understand the patient’s needs and making sure any questions are answered
14. Maintain a safe environment by writing instructions and orders clearly, obtaining a second opinion from one of your colleagues if necessary, asking for assistance, knowing how to use all equipment and treating the equipment well
15. Provides a positive leadership role model to the dental staff, portraying the expectations and culture for the dental operation, consistent with OLV Values, empowering others to initiate new ideas and be accountable
16. Any other duties as assigned by supervisor

Minimum Education Required:
Completion of an accredited program for Nurse Practitioners
Minimum Experience Required:
1 year experience
*Previous experience in areas of Developmental Pediatrics, Orthopedics, PM&R, genetics or Neurology preferred
License/Registration/ Certifications Required:
NYS Driver’s License
NYS Nurse Practitioner License
