Title:              Clinical Coordinator

Salary:           $55,000 - $57,000

 

Position Overview:

Under the general direction of the Director of Social Services, with some latitude for independent action and decision making, the Clinical Coordinator is responsible for performing key components of the client intake process. The Clinical coordinator will work evening hours, in order to engage clients as they enter the facility. 

 

Program Overview:

Kenton Hall is an emergency men’s shelter which helps 108 individuals with substance abuse histories work towards health, sobriety, and, ultimately, permanent housing and employment.  In addition to lodging and three meals a day, clients receive comprehensive case management services.

Essential Duties & Responsibilities

  • Responsible for the initial intake and assessment of every new arrival with respect to housing readiness.
  • Write new intake or returnee case notes
  • Audit minimum of 6 charts per week and provide detailed summary to DSS following the audit.
  • Facilitates a direct hand-off to nursing to ensure the initial nursing and psychiatric assessments are initiated.
  • Reports daily to the Director of Social Services on the status of all new clients.
  • Coordinates with the Director of Social Services and Housing Specialist to identify and expedite placement for clients who may have other housing options.
  • Assigns new clients to case manager within 48 hours of their arrival.
  • Completes initial psychosocial within a month of client’s date of entry.
  • Manages and updates caseload breakdown daily.
  • Provides guidance and direction to Housing Specialist ensuring 2010e packages are being generated.
  • Provides support to case management staff ensuring ILPs, Incident follow-ups, and charts are up to PRI and DHS standards.
  • Assists with the facilitation of case conferences.
  • Maintain program’s statistical data
  • Acts as liaison with local hospitals and treatment centers.
  • Follows up with clients who have been in serious incidents.
  • Tracks psychiatric and medical ER visits/hospitalizations monthly.
  • Keeps track of clients on ACT/AOT/HUB.
  • Provides support to clients in the absence of their case manager.
  • Schedule medical and psychiatric appointments.
  • Submits incidents in WeShare as needed and maintains incident reporting spreadsheet to ensure that follow-ups are captured.
  • Updates hospitalization and chart audit trackers.
  • Attends and participates in the monthly P4P Case Conference meetings on the status of each client.
  • Performs other duties as assigned by the Shelter Director and Director of Social Services.

Qualifications:

 

  • Master’s Degree in Social Work Preferred
  • Minimum of 3 years’ experience working with homeless mentally ill/MICA adults
  • An ability to motivate all staff and to work collaboratively with staff from other programs to provide the most beneficial services to the clients.
  • Excellent oral, writing and listening skills.
  • An ability to work independently and to initiate actions to support the efforts of the staff and the clients.
  • An ability to relate with clients as well as all levels of staff.
  • Knowledge of case management software as well as a proficiency in Microsoft Office Suite.

 

Physical Activities:

 

Job duties generally require the physical activities described below, subject to any necessary reasonable accommodation due to disability. While performing the duties of this position, the Clinical Coordinator must be able to walk between five floors of the facility. In addition, the Clinical Coordinator must stand, bend in order to use the files and be able to sit at the computer workstation for extended periods to review the client data and update the information in the case management systems. Further, the Clinical Coordinator must be able to lift or move material and equipment weighing up to 20 pounds.

 

Language: Spanish speaking preferred.
