Title:    Licensed Practical Nurse

Location: Support and Connection Center (116th St. Manhattan)

Schedule: Per Diem– Days 12hrs. Schedule flexibility, including weekends.  

Rate: $40.00/hr

 

Position Overview: 

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at The Support and Connection Center. The Support and Connection Center offers medical and social services to individuals affected by homelessness, mental health challenges, and substance use. The LPN reports directly to the Nurse Manager of the Support and Connection Center; indirectly reporting to the Director of Nursing Operations, and the Program Director of the Support and Connection Center 

 

Essential Duties & Responsibilities:  

  • Direct Client Care: 

  • Conduct comprehensive health screenings for clients with substance use disorders and co-occurring mental health conditions, considering physical, mental, and social factors. 

  • Administer medications, assure safe storage of medications, maintain accurate documentation of medications, assists clients with self-administered medications, provide medication teaching. 

  • Education and Counseling: 

  • Educate clients on harm reduction practices, overdose prevention, and mental health self-care strategies. 

  • Collaboration and Coordination: 

  • Work collaboratively with multidisciplinary teams including psychiatrists, social workers, occupational therapists, peer advocates, and rehabilitation aides. 

  • Coordinate referrals to community resources such as addiction treatment centers, mental health services, medical services, and housing support. 

  • Advocate for clients within the healthcare system to ensure integrated and comprehensive care. 

  • Documentation and Reporting: 

  • Maintain documentation in compliance with agency policies and procedures 

  • Participate in team meetings to discuss integrated client care and treatment outcomes. 

 
 
 

Qualifications: 

  • Working with unhoused individuals preferred. 

  • Community Health settings preferred.  

  • Compassionate, non-judgmental approach to healthcare delivery. 

  • Experience or strong interest in working with individuals affected by substance use disorders and mental health conditions. 

  • Knowledge of harm reduction principles and practices. 

 

Education Credentials/Requirements: 

New York State LPN  

 

Preferred skills:  

Communication: Must have excellent oral, listening, and writing skills.   

Interpersonal skills: Must have the ability to interface with clients as well as all levels of multidisciplinary staff.  

Computer skills:  Must have knowledge of Microsoft Office. 

 

 

All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
