Licensed Vocational Nurse-Caregiver

Job Purpose:
Provides advanced level of home health care to Tribal Members with chronic disease or serious illness requiring advanced medical support for medications, feeding, and monitoring of disease. Works under the direction and supervision of Sycuan Medical Clinic Supervisor and/or Executive Director assigned by Sycuan Band of the Kumeyaay Nation, and the client’s Primary Care Physician.
Policy: Licensed Vocational Nurse must perform only duties that are within limitations of the scope of practice and in accordance with the guidelines stipulated by the Board of Vocational Nursing. It is the responsibility of the LVN to know his/her limitations and abide by the regulatory standards. LVNs are responsible for maintaining clinical competence with regard to those skills and activities expected of a LVN. At no time should an LVN perform an activity in which he/she does not feel clinically competent or questions the legal scope of practice for that activity.
Scope of Practice: In California, Licensed Vocational Nurses practice the performance of services requiring those technical, manual skills acquired by means of a course in an approved school of vocational nursing, practiced under the direction of licensed physician, mid-level provider, or registered professional nurse. Please see Essential Duties/Responsibilities below for specific procedures LVNs are able and unable to perform.
Summary: Works under the supervision of the Sycuan Medical Clinic Supervisor and/or Executive Director and client's Primary Care Provider [PCP]. Provides excellent patient care to the Sycuan Band of the Kumeyaay Nation Tribal Member. The LVN/caregiver will plan and follow a daily care schedule with the assigned Tribal Member, modify care plans as prescribed by medical professionals, and report any new conditions or needs that may arise. The LVN/caregiver will be patient, empathetic, observant, and committed to making Tribal Member feel safe and comfortable.
Job Duties and Responsibilities:

- Assists with activities of daily living such as bathing dressing and bed and/or vehicle transfers
- Provides daily meals, according to nutritional needs and diagnosis requirements
- Prepares and administers medications ordered by Primary Physician via Percutaneous Endoscopic Gastrotomy tube.
- Assists with managing PEG tube maintenance [cleaning, report signs of infection if present]
- Willing to engage in recreational game activities with client
- Reports any significant changes in patient’s condition to supervisor and Primary Care Physician
- Transports patient as necessary
- Adheres to standards for infection control in accordance with local, state, and federal OSHA regulations
- Maintains privacy and confidentiality
Job Specifications:
Education and Experience:
Essential:
- Active and current License as Vocational Nurse in the State of California
- Current CPR certification
- CA driver’s license in good standing
Desirable:
- Tribal clinic experience preferred
- CNA certification preferred
Skills and Knowledge:
Essential:
- Excellent English oral and written communication skills
- Ability to prioritize and perform multiple tasks
- Ability to administer medications safely via PEG
- Ability to care for PEG
- Ability to maintain confidentiality
- Ability to appear for work at scheduled time
- Ability to maintain professionalism and composure

Physical/Mental:
- Able to lift/move up to 40 pounds, move from place to place, and stand for long period of time
- Ability to organize and prioritize workload, work effectively and efficiently under stress
- Ability to understand and follow instructions
- Ability to proficiently read, write, speak and understand English

- Immediately reports unsafe working conditions
- Follow all infection control procedures
