About this opportunity:

This position provides individual, group counseling in relation to the treatment and recovery of Veteran clients; develops treatment plans and participates in treatment team decisions; prepares and maintains accurate and up-to-date client records and progress reports; serves as liaison to other staff to maintain awareness of client’s behavior and progress and needs.  Meets with other case management staff to coordinate treatment objectives. Refers clients to community resources as appropriate. Supervises case management staff in the Veterans Program.

Key responsibilities:

  • Interviews clients and conducts assessments; produces written assessment outlining substance abuse history, personal history, testing results, impressions, and recommendations; consults with Case Management staff and Program Coordinator regarding assessment results and recommendations.
  • Develops individualized treatment plans and maintains updated revisions of each plan in relation to the progress of each client; ensures that treatment plans address the physical, emotional, vocational, social, and spiritual needs of each client with identified time limits and measurable objectives.
  • Conducts counseling sessions; conducts individual sessions for clients in office, utilizes various counseling techniques including, but not limited to motivation interviewing, cognitive therapy techniques; coordinates for more intensive treatment if needed.
  • Assists individual with managing and preventing crisis.
  • Outsource clients to additional counseling resources if needed.  Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual client.
  • Maintains community contact with appropriate agencies for additional and supplement services and referrals which will assist in meeting the needs of each client.
  • Facilitate group and individual counseling to ensure clients’ behavior and motivations are progressing, promote self-awareness and provide appropriate encouragement.  Use evidence-based practices, such as Cognitive Behavioral Therapy, Seeking Safety, Trauma Resiliency Empowerment Model, etc.
  • Plans, coordinates, and/or facilitate, behavior modification, addiction related (Celebrate Recovery), etc. classes to assist clients in obtaining skills that will enable them to functionally cope with their environment.
  • Prepares and maintains client files; produces written assessment reports with impressions and recommendations, make contact notes on all counseling sessions, maintains all needed forms for consent, goal planning; documents life sills attendance; maintains case files and all paperwork needed for case managed clients.
  • Participates in staff meetings, reviews client issues with specific cases; makes recommendations; case managers; participate in general case reviews during staffing and other program business.
  • Monitors treatment program to ensure compliance to state practices.
  • Facilitate MDT (Multi-Disciplinary Team Meeting) with Macon staff and clients.
  • Supervises and manages case management staff in the Veterans Program.
  • Participates in community planning for Veterans issues; to coordinate and network Veterans services in the local community.
  • Ensures that services are delivered to participant using a client-centered, strengths-based approach.
  • Maintain strict confidentiality regarding all Veterans participants; adheres to HIPAA guidelines/relations.
  • Attend all in-service Professional Development training sessions.
  • Attends scheduled staff training and educational workshops.
  • Performs other related work as required.
  • Follows rules, regulations, and policies of the Department of Social Services and The Salvation Army.

What we are looking for in you:

  • Bachelor’s Degree in Social Work, Licensed Social Worker in the State of GA. Master of Social Work preferred.
  • Two years’ experience in a licensed substance abuse program
  • Any equivalent combination of education and experience which provides the required knowledge, skills, and abilities. Experience working with Veterans is preferred.
  • Licenses by Georgia Board Composite Board of Professional Counselors, Social Workers, and Marriage and Family Therapist as LPC or Master of Social Work – Advanced Clinical Practitioner (LCSW) or LMFT Chemical Dependency Counselor (LCDC)
  • Valid State Driver’s License.
  • Must be able to obtain and maintain driving privileges per Salvation Army Insurance standards.

 

Physical Requirements and Working Conditions:

Ability to meet attendance requirements, ability to read, write and communicate the English language, ability to operate and answer the telephone, ability to stay alert and awake while on duty.  Limited amount of physical effort required associated with walking, standing, lifting, and carrying light objects (less than 25 lbs.) 5-10% of work time.  Duties are usually performed seated.  Sitting may be relieved by brief or occasional periods of standing or walking.
