THE ROLE: The Case Manager provides case management services to newly arrived refugees, SIV, and granted asylees. Culturally responsive trauma informed case management is focused on supporting clients with complex medical issues with the goal of self-sufficiency for the client in navigating the complex medical system. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

Under the direction of the Program Manager, the Case Manager will:

  • Provide culturally and linguistically appropriate case management services to qualified clients dealing with complex medical issues.
  • Assess and address other needs relating to the social determinants of health and provide case management services to address those needs.
  • Conduct home visits, attend medical appointments, and accompany clients as appropriate to other community provider appointments.
  • Ensure appropriate documentation and gather outcome data under the direction of the Program Manager.
  • Provide workshops to eligible individuals on how to navigate the medical system.
  • Conduct self-sufficiency workshops for medically complex individuals.
  • Develop and maintain relationships with refugee communities, resettlement agencies, mutual assistance associations, and other community-based organizations.
  • Work effectively with community members (staff, clients, doctors, agencies, etc.) to reduce cultural and socio-economic barriers between clients and institutions.
  • Communicate information to clients and providers clearly and succinctly, both written and verbal.
  • Ensure clients assigned are contacted within the timeframe and receive appropriate services and follow-ups.  
  • Requires strict adherence of HIPAA standards in the office workspace to ensure confidentiality.
  • Other duties as assigned.

 

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

 

Minimum Qualifications

  • Bachelor’s degree or equivalent work experience in relevant field.
  • Minimum of 1 year working in a case management or social service role.
  • Minimum of 2 year working with refugee populations.
  • Bilingual in English and Dari/Pashto/Farsi/Arabic

 

LANGUAGE SKILLS: Ability to read, write, and understand directions.  Must have the ability to write memos and other basic correspondence.  Ability to effectively present information and respond to questions from employees, clients, and the general public.  Must have excellent communication skills with the ability to read, write, and speak English with one additional language.

 

CERTIFICATES, LICENSES, REGISTRATIONS: Frequent travel in King County. Possible travel in Pierce County. Requires a valid Washington State Driver’s License and auto insurance.
