Reports to: Program Manager, Santa Monica C3
Status: Full-time, Non- Exempt (Hourly), Non-Management
Location: Santa Monica Old Firehouse
Schedule: Monday- Friday 8:00am-4:30pm

JOB SUMMARY

With the support of and in partnership with the Santa Monica C3 Program Manager and Westside Outreach leadership, the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in high density areas of Santa Monica. The Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.


Essential Duties and Responsibilities 

1. Work as part of an interdisciplinary team to engage and provide services to homeless individuals.
2. Interface and represent the agency with partners, including community health providers, the business community, and social service/housing providers to reduce the number of homeless people living on the streets.
3. Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition.
4. Provide case management and aftercare/follow-up services to clients placed in permanent housing.
5. Report all statistics and updates for reporting purposes.
6. Provide initial contact, needs assessment, information, education and referrals to the homeless.
7. Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.).
8. Develop effective, trusting relationships with clients that facilitate provision of services.
9. Maintain and record accurate electronic records, client files, and statistical information.
10. Attend all case file, staff meetings, trainings and retreats.
11. Work as part of a larger team composed of all Santa Monica Outreach interdisciplinary teams.
12. Physically able to participate in field outreach in multiple locations.


QUALIFICATIONS


1. BA degree or equivalent experience.
2. Spanish speaking preferred.
3. Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
4. Working knowledge of social services agencies in West LA (SPA 5) and City of Santa Monica agencies serving the homeless.
5. Excellent human relations/communication skills; ability to act in professional manner regardless of circumstances.
6. Skilled in non-violent crisis intervention.
7. Ability to self-motivate and organize time effectively and efficiently.
8. Computer literate; able to use computerized database system for information management.
9. Passion for working with the population served.
10. Ability to work as part of an interdisciplinary team


Preferred Qualifications:


1. BA degree or equivalent experience.
2. Spanish speaking preferred.
3. Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).


WORK ENVIRONMENT:

1. Combination of field and office environment; shared office space with stairs.
2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
3. On occasion walk or drive to different local sites.
4. Must be able to walk outside for 1-2 miles and conduct outreach in different weather conditions.
5. Will necessitate working in busy and loud environments.
