Job Title: Support Specialist, Housing
Department:  Housing
Reports to: Assistant Director, Project Based Housing
Location:  6th Street Place, 401 E. 6th Street, Los Angeles, CA  90014
Schedule:  Tuesday to Saturday, 12:30 PM to 9 PM
Status:       Full-Time, Non -Exempt, Non-Management

SUMMARY

The Support Specialist will function as part of a multi-disciplinary team and provide site-based therapeutic and intervention services to clients residing in Project-based Housing. These services include supporting the LVN by performing various care activities and by providing assistance with activities of daily living (ADLs). In addition, the Support Specialist will assist Case Managers in working toward goals created on each individual services plan to eliminate barriers to housing and health stability. The selected candidate will also be responsible for monitoring daily site activities and conducting wellness groups to create a sense of community and increase life skills.


ESSENTIAL DUTIES & RESPONSIBILITIES 

1.    Support orientation to permanent housing for new clients.
2.    Oversee housekeeping and hygiene guidelines in a trauma-informed manner.
3.    Work with LVN to support medical intervention plans.
4.    Collaborate with Case Managers to ensure appropriate and proactive client care.
5.    Work in collaboration with other Project-based Housing staff to maintain smooth operation of the housing site, which may include assisting with meal support, or other duties as assigned.
6.    Order office and program supplies, when needed.
7.    Facilitate client groups to assist with life skills development.
8.    Develop effective, trusting relationships with clients using a client-centered approach that includes motivational interviewing, harm reduction, and trauma-informed care.
9.    Proactively create an inclusive and engaging environment to support clients’ permanent housing retention and psycho-social rehabilitation.
10.    Work with on site TPC leadership and supervisory staff to develop an effective working relationship with property management.
11.    Assist Property Management with client-related requests when appropriate.
12.    Intervene during crises to actively de-escalate situations, supporting safety for both staff and clients.
13.    Communicate effectively, and in a timely manner, with management, co-workers, and clients.
14.    Always maintain client confidentiality according to agency, state, and HIPAA laws.
15.    Report all statistics and outcome updates, as requested, for reporting purposes.
16.    Attend all case conference, staff meetings, and department trainings.
17.    Other duties as assigned.

QUALIFICATIONS/REQUIREMENTS:

1.    High School diploma or GED strongly preferred 
2.    Minimum of one year experience providing services to individuals that have a history of homeless, mentally illness, or are dually-diagnosed 
3.    Demonstrated knowledge of issues faced by a culturally diverse population
4.    Proficiency In Word, Excel and Outlook required


WORK ENVIRONMENT:

1.    Field (may need to travel) and indoor office environment
2.    On occasion walk or drive to different local sites
3.    Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds
4.    Will necessitate working in busy and at times loud environments
5.    Will be exposed to elements like cold, heat, dust, noise and odor
6.    May need to bend, stoop, twist, and sit throughout the day
7.    Able to thrive in a work environment emphasizing teamwork and collaboration
