Recuperative Care Case Manager

As an RCP Case Manager, you will be a vital member of our team, working closely with homeless individuals who require 24-hour supervision following a hospital discharge. You’ll serve as the single point of contact for healthcare facilities, ensuring that our clients receive the care and support they need to stabilize their physical health and transition to a more secure environment.

We would love to hear from someone who:

  • Has a Masters Degree in Public Health Social Work, Psychology, OR related field, OR
  • Has a Bachelor’s Degree in Social Work, Psychology, or related field.
  • Is a Certified Drug/Alcohol Counselor.
  • Has experience in medically related direct client services, including assessment, evaluation, treatment/discharge planning, referral and follow-up to a population comprised of homeless individuals, parolees or probationers, the mentally ill, the chronically ill, or high-risk youth. 

As a Case Manager, you will:

  • Facilitate the transfer of clients to 40 Prado, our shelter, and work with staff to provide essential services.
  • Complete intakes and assessments and develop individualized case plans alongside our program RN.
  • Monitor clients' progress through daily contact and help them move toward greater physical self-sufficiency.
  • Coordinate services with public and private agencies to ensure continued client stability.
  • Interface with healthcare facilities to assess and determine client appropriateness for RCP services.
  • Develop and monitor individualized care plans with both short- and long-term goals.
  • Maintain strong, collaborative relationships with healthcare providers, community partners, and other agencies.
  • Advocate for clients to ensure they receive the medical, mental health, and other services they need.
  • Assist clients in transitioning out of the RCP and into the general shelter population or other housing options.

Additional Eligibility Qualifications:

  • Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual  orientation, disability or socio-economic level.
  • Must be at least 18 years of age by date of hire. 
  • Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or sign a COVID-19 vaccination declination form.
  • Based on program, a current TB skin test/Chest X-Ray may be required. 
  • Employment is contingent upon meeting all applicable job requirements and background requirements. A criminal history clearance is required prior to employment. 
  • Upon hire, agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment. Agency staff will check the website annually to ensure all program staff are cleared to work.
