Job Title: Case Manager 

 

Salary Range:$52,000-54,000

 

About the Position

The Case Manager is responsible for making an in-depth assessment of students’ needs and determining what resources are available to assist them. The Manager is responsible for managing caseloads, referrals, goal setting and acts as an advocate/facilitator to help meet student needs. 

 

Essential Duties and Responsibilities include the following.  Other duties, responsibilities, and activities may change or be assigned at any time with or without notice.

  • Case manages students to reduce barriers and meet basic needs, providing services for needs through Goodwill or by referral.  
  • Provide one-on-one services, workshop content, and referrals to students via in-person meetings or remote service delivery via video conferencing. Provide workshops on basic needs and resource topics regularly. 
  • Provide advocacy, and follow up with providers regarding referrals. Troubleshoot problems that arise and provide solution-focused ideas to assist referrals in accessing support services, and current clients in maintaining eligibility for support services.
  • Complete intake assessment including all required forms and documents for student's enrollment. 
  • Maintain clear and accurate case notes in database and safely maintain paper and electronic student files. Collect, monitor, and report on student outcome data if applicable.
  • Build professional relationships with students and Center staff that facilitate successful outcomes. 
  • Collaborate with students, staff, and partners to implement student-centered initiatives.  
  • Present pertinent case-related information via reports or meetings. 
  • Actively participate in staff meetings and other departmental projects. 
  • Network with community-based agencies, organizations, and groups to stay informed about area services and resources if applicable. 
  • Demonstrates safe work practices through awareness and observation in support of a safety culture; reports any potential hazards or accidents. 
  • Perform other duties as assigned. 

 

Position Requirements

Education and/or Experience:

  • Master’s or Bachelor’s degree in Social Work or related field or equivalent combination of experience and education. Master’s Degree preferred; however, lived experience and international experience considered in lieu of more traditional, U.S. achieved credentials. 
  • Minimum two (2) years of social work, human services or case management experience required.   
  • 2-3 year’s social services experience involving interviewing, counseling, or crisis intervention.
  • Knowledge of local community resources and agencies, as well as government benefit and immigration policies and procedures.  
  • Experience with remote service delivery a plus.  
  • Ability to communicate and work effectively with students from a variety of economic, cultural, and ethnic backgrounds, physical and mental abilities and sexual orientations and those with limited English skills.  
  • Multi-lingual a plus.  
  • Must demonstrate a high level of commitment to equity and non-discrimination in practice and execution of duties. 
  • Ability to read and interpret documents such as law enforcement reports, government or agency regulations and legal interpretations.  
  • Ability to practice confidentiality.  
  • Skilled in de-escalation techniques. 
  • Ability to work independently and as part of a team. Ability to make good decisions without on-site supervision.  
  • Remote and occasional evening and weekend work required.
  • Must be able to meet travel requirements of up to 20% of work time locally and statewide.

 

Certificates, Licenses, or Registrations:

  • Must have valid driver’s license and proof of insurance.
  • CPR/First Aid certification preferred. 

Computer/Technology Skills:

  • Highly proficient with web-based applications (Google Workspace), video conferencing, case management software, and Microsoft Office.
  • Ability to learn new technology and software as related to the job.  

Physical Demands:

While performing the duties of this job, the employee will be required sit for long periods of time, communicate with other employees by talking and hearing and to operate computer systems. Able to safely lift/push/pull up to 25 lbs., grasp, handle, finger and manipulate items; perform repetitive fine motor functions with fingers, wrists, and arms, standing for long periods of time. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Able to communicate with others effectively both verbally and written, including those who have limited English skills. Maintain reliable attendance.

Work Environment:

Office/classroom environment; the noise level in the work environment is usually moderate. Must have reliable transportation, as this position requires travel to offsite meetings and events.
